"Can acupuncture actually improve my IVF success rate?" It is the question we hear most often from patients navigating assisted reproductive technology — and it deserves a careful, honest answer rather than a sales pitch.

The short answer: the evidence is promising but not definitive, the proposed mechanisms are biologically plausible, and the risk-to-benefit ratio strongly favors trying it. Here is a deeper look at what the research actually shows.

What the Studies Show

The most-cited early studies — published in Fertility and Sterility in 2002 by Paulus et al. — found that acupuncture performed 25 minutes before and after embryo transfer significantly increased clinical pregnancy rates compared to a control group (42.5% vs. 26.3%). This sparked a wave of follow-up research.

Subsequent studies have produced mixed results, which is not unusual in a field where patient populations, acupuncture protocols, and IVF protocols vary enormously. A 2018 JAMA meta-analysis of 17 randomized controlled trials found no statistically significant improvement in live birth rates from acupuncture on the day of embryo transfer alone — but this analysis has been criticized for pooling studies with vastly different protocols and for using sham acupuncture controls that may themselves have physiological effects.

More recent research has shifted focus from single-session transfer-day acupuncture to multi-week treatment protocols that begin weeks before retrieval — and these studies show more consistent positive trends. A 2020 systematic review in Reproductive BioMedicine Online found that acupuncture administered throughout an IVF cycle (not just on transfer day) was associated with improved clinical pregnancy rates.

Proposed Mechanisms

Several biologically plausible mechanisms have been proposed to explain how acupuncture might support IVF outcomes:

  • Uterine blood flow — Acupuncture has been shown in multiple studies to increase uterine artery blood flow, measured by Doppler ultrasound. Improved endometrial perfusion supports lining development and implantation.
  • Stress hormone regulation — IVF is profoundly stressful, and elevated cortisol directly suppresses reproductive hormone signaling. Acupuncture activates the parasympathetic nervous system and has been shown to reduce cortisol and beta-endorphin levels.
  • Ovarian response — Some research suggests acupuncture may improve ovarian response to stimulation medications, potentially increasing the number and quality of retrieved eggs.
  • Uterine receptivity — Acupuncture may modulate the immune environment of the uterus, supporting the immunological tolerance required for successful implantation.

How We Time Treatments

Dr. Mason's approach to IVF support is protocol-specific and coordinated with your reproductive endocrinologist's timeline. A typical treatment schedule looks like this:

  • Pre-cycle (4–8 weeks before): Weekly acupuncture to optimize baseline hormonal balance, uterine blood flow, and stress levels. Herbal medicine may be used during this phase (paused once stimulation begins).
  • Stimulation phase: Acupuncture 1–2 times per week to support ovarian response and manage side effects of stimulation medications.
  • Retrieval day: Acupuncture before retrieval to reduce anxiety and support recovery.
  • Transfer day: Acupuncture before and after transfer to maximize uterine receptivity and reduce uterine contractions.
  • Two-week wait: Acupuncture 1–2 times to support implantation and manage anxiety.
  • Post-positive test: Continued support through the first trimester if desired.
